Foundations of Transplant
- Basic concepts of transplantation: Hematopoietic cell lineage and function, Immune system and function, Histocompatibility, Cellular goals of therapy (e.g., graft-versus-tumor effect, hematologic/immune reconstitution). Hematologic indications for transplantation (malignant and non-malignant). Types of transplants: Autologous, Allogeneic (MRDs, MURDs, mismatched unrelated donors, umbilical cord, haploidentical), CAR-T. Sources of stem cells: Peripheral blood, Bone marrow, Umbilical cord. Recipient suitability and evaluation, Recipient education, Caregiver education, Donor selection, care, and education.
Transplant Process and Infusion
- Cellular therapy product collection and storage: Stem cell transplant product mobilization, collection, harvest, and storage. Conditioning/preparative regimens: Intensity of therapy, Chemotherapy, Radiation therapy, Biotherapy, Immunotherapy, Targeted therapies. Management of acute complications related to preparative regimens. Cellular infusion: Fresh vs. cryopreserved, Infusion management, Hematologic compatibilities. Cellular therapies (e.g., donor lymphocyte infusion, CAR T).
Early Post-Transplant Management and Education
- Immunosuppressive therapy, Acute graft-versus-host disease (GVHD), Infection prevention and management, Sepsis. Hematologic (e.g., engraftment, pancytopenia, transfusion support), Immune reconstitution (e.g., engraftment syndrome, cytokine release syndrome), Nutritional support, Acute system-specific complications (e.g., veno-occlusive disease; gastrointestinal), Graft rejection or failure, Chimerism. Symptom management for alterations in physiologic function (e.g., pain, nausea, vomiting, fatigue).
Late Post-Transplant Management and Education
- Chronic graft-versus-host disease (GVHD): Medical management, Photopheresis. System-specific late effects (e.g., bronchiolitis obliterans, cataracts, infertility). Infection prevention and management (e.g., immunizations), Disease relapse, Subsequent malignancy, Follow-up care and milestone visits.
Quality of Life
- Navigation and coordination throughout the continuum, Psychosocial (e.g., coping, family and caregiver support), Health promotion and maintenance, Sexuality, Cultural and spiritual competence, Survivorship, Palliative care, End-of-life care (e.g., hospice, legacy building).
